What do I do with the form?

Take or send the filled-in form and fee to the court where the judgment was entered (made). If the claim has been transferred since judgment, you should take or send it to that court. The court will issue the warrant within 5 to 10 working days of receiving your form and fee.

If you want confirmation that the warrant has been issued, and to know the warrant number, you should also send a stamped addressed envelope.

If the defendant sends you any payments after you have sent the form to the court, you must tell the court immediately.
