Before you fill in a form
Read through the form and plan your answers. Remember:
- if you do not want someone to know your address, or the child's, you do not have to put the address on the forms. But you must still give the address to the court. Use form C8 to do that.
- do not refer to:
adoption orders, or parental orders made under Section 30 of the Human Fertilisation and Embryology Act 1991.
- if you are applying during a divorce put the case number of the divorce proceedings, and the name of the court, in Part 1 of form C2.
When you fill in a form
Use dark ink (because you will have to make copies of the forms).
If you need more space to give the information, use another sheet of paper and put at the top of it:
- the child's name
- the child's number or the case number, if you know it (see Page 3)
- the number of the part you are answering.
Please give as much information as you can. If you do not know something, or if you think a part does not apply to you, say so. If you do not answer a part of a form the court may ask you why.
Remember the court staff cannot help you decide what to put on the forms, or give you any advice about your case.
